The EZ 2000 Kiosk allows patients to complete forms electronically at any computer in the dental office. 

You can setup a workstation that is used as a Kiosk only, or run the Kiosk on a computer that is also used for other purposes.

The Kiosk is useful when you want a patient to complete forms in the reception area or in an operatory. 

It allows patients to sign in electronically complete Health Histories, Patient Registration forms, HIPAA Acknowledgements, Medical Histories, Financial Agreements, and more.

Please choose remote (free) installation or onsite installation(charge).

Note that onsite installation does not include cat5 cabling which you are required to pay for separately.
